COVID-19 Vaccine Clinic for Ages 5 – 11

This clinic is only offered to Punahou families with children ages 5 – 11 (including non-Punahou student siblings). Pre-registration is required. Walk-ins will not be accepted at this clinic.
 
For those who scheduled an appointment for the clinic on Nov. 12, please review the pre-vaccination checklist from Hawaii Pacific Health which you received upon registration. This will inform you of what you need to bring with you to the clinic.
